How many people are you supposed to hold a door for?

When you are walking into a building and you graciously hold the door open, how many people can filter through before it's your turn. 2, 4, 6, the entire building's employee list - where exactly is the cut off, because eventually your arm gets tired.

And for that matter, how many people do you have to let in before you can walk out? Etiquette says when people want to enter and leave at the same time, the person exiting has the right of way. How come all the people coming to Cosi on 45th today didn't know that? And what makes their sandwiches so tasty that the hoards of starving midtowners had to stampede in?
